In January 2003, the VA system apparently adopted a directive to prevent such wrong surgeries from occurring. Basically, these safety procedures require surgeons to implement a system of checks involving the patient’s identity, test results, the procedure to be performed and where exactly it is to be performed. The surgical team, based on this directive, also took a “pre-op timeout” for one last check. Not surprisingly, these steps helped prevent wrong surgeries. Other studies have estimated that between five and 10 wrong surgeries occur every day in the United States.
